What is Form DS-82?
Form DS-82 is the U.S. Department of State application for renewing a passport by mail. Eligible U.S. citizens can use it to renew a passport book, passport card, or both. It is not the right application for every passport renewal.
You may qualify if your most recent passport is undamaged, was issued within the past 15 years, and was issued when you were age 16 or older. Other rules may apply, such as having the passport in your possession and meeting the name requirements listed on the application.
Using Form DS-82 lets eligible applicants renew without visiting a passport acceptance facility. This can reduce the time and effort involved in keeping a valid passport for international travel. Review the State Department instructions before you complete and mail the application.
What is Form DS-82 used for?
Form DS-82 lets eligible U.S. citizens renew an expiring or expired adult passport book or passport card by mail. It records your current personal details, mailing address, and emergency contact information for the new passport. You can also report a legal name change due to marriage or a court order by including the required supporting document.
You must provide your Social Security number as required by federal law. By signing and dating Form DS-82, you certify that the information you gave is true and correct under penalty of law.

Where to get a blank Form DS-82
The U.S. Department of State issues Form DS-82 through its Bureau of Consular Affairs. You can get an official blank copy from the passport forms page on travel.state.gov. If you qualify for online passport renewal, you may not need a printed form. Renewal by mail requires a printed Form DS-82.

Where to send Form DS-82
If you completed and downloaded Form DS-82, print and mail it with the required documents and payment to the U.S. Department of State’s National Passport Processing Center. Use the mailing instructions provided with the form, since the destination may depend on your location and requested service.
Do not take Form DS-82 to a passport acceptance facility. If you qualify for online renewal, you may submit your application through the State Department’s official online renewal system instead of mailing the downloaded form. The State Department is the only authorized online renewal portal, and not all applicants qualify.
Who needs to complete Form DS-82?
Adult U.S. passport holders complete Form DS-82 if they qualify to renew by mail. Your latest passport must be undamaged, in your possession, and not reported lost or stolen. It must have been issued when you were age 16 or older, within the last 15 years, and in your current name. If your name changed, you must provide proper proof. If you do not meet every rule, apply in person with Form DS-11 instead.
After you mail the completed form with your old passport, photo, and fee, the National Passport Processing Center uses it to confirm your eligibility and process the renewal. The U.S. Department of State then issues the new passport book or card and returns the old passport separately.
How to sign Form DS-82

What information/documents are required?
Have your most recent U.S. passport book and/or card, one passport photo, and any certified document that proves a legal name change. For Form DS-82, enter your name as it should appear on the new passport, then your date of birth in month/day/year format. Provide your email address and phone number in item 6.
For mailing address, include your street, P.O. Box, or rural delivery name, apartment number, city or town, state abbreviation, and foreign country if needed. List all other names used, including maiden, birth, alias, or former married names. Attach another sheet if two lines are not enough. For name change information, give the details and date in mm/dd/yyyy format, and include certified proof if your name changed. Add travel plans if applicable. After printing, sign and date the form, in an authorized person’s presence if your submission method requires it.
When is Form DS-82 needed?
Form DS-82 has no fixed due date. You can submit it when you need to renew, as long as your latest passport was issued within the past 15 years. It must also be undamaged, in your possession, issued when you were age 16 or older, and in your current name. If your name changed, include legal proof of the change.
Renew before planned international travel. Many countries require your passport to remain valid for at least six months after your trip, so applying early can help you meet entry rules.
When is Form DS-82 not required?
Form DS-82 is not required when you cannot submit your most recent passport with your application. You must also use Form DS-11 and apply in person if that passport was issued before you turned 16, was issued more than 15 years ago, or is damaged, lost, or stolen.
Use Form DS-11 if your name has changed and you cannot provide legal proof of the change. In some cases, another application may apply. For example, you may qualify to use Form DS-5504 to correct a passport printing error. Check the U.S. Department of State rules for your situation before preparing your application.
