It also sets goals and an improvement plan, giving the leader a clear basis for follow-up counseling and progress checks over time. The discussion may cover possible effects on a Soldier’s military career, bonus, pay, education, medical care, or employment.

Who needs to complete Form DA 4856?
Army leaders and NCOs complete Form DA 4856 when they counsel a subordinate. This includes raters, first-line supervisors, and other leaders in a leader-subordinate relationship across all ranks and Army components. The counselor records the session, and both the counselor and the Soldier being counseled sign the form.
Afterward, the chain of command and evaluation raters may use the record to support OER and NCOER documents. Promotion boards and military school selection boards may review counseling records in a Soldier’s file. The form may also be reviewed during administrative separation actions or other formal proceedings.

DA Form 4856 does not list a required supporting document, but relevant records may help support the counseling details.
When is Form DA 4856 needed?
Form DA 4856 has no fixed universal due date. Complete it whenever developmental counseling takes place, either during the session or right after it.
Initial counseling is generally required within 30 days of starting a new duty position or rating period. Later counseling may occur monthly or at other intervals set by unit or NCOSP policy. Events such as performance issues, promotions, reassignments, or a fitness test failure may also trigger counseling. Follow the commander’s guidance, AR 623-3, and your local standard operating procedure for the required timing.
When is Form DA 4856 not required?
Form DA 4856 is not required when no military leader-subordinate relationship exists. This includes civilians with no military affiliation. Military regulations do not place counseling documentation duties on them. Retirees also generally fall outside this requirement because the form applies mainly to active-duty members and reservists.
ATP 6-22.1 provides no blanket exemption based on rank or duty position. It also sets no numeric threshold that removes the need for the form. The guidance does not name a substitute form or allow a third party to complete it in place of the responsible leader.
