Next, gather the Child's Name, Date and Place of Birth, plus details about the Child's Current Residence, including where and with whom the child primarily lives. You also need the deployment dates for paragraphs a and b and the Temporary Physical Guardian's Name.
The Soldier must provide the SOLDIER'S SIGNATURE. The other parent must complete the OTHER PARENT'S SIGNATURE before a notary. The notarization block requires STATE OF, COUNTY OF, the acknowledgment date, NOTARY PUBLIC, and My commission expires. The form does not list a separate supporting document, but the other parent should ask the notary what identification is needed for notarization.
When is Form DA 7666 needed?
Form DA 7666 has no separate fixed deadline. Submit it with your Family Care Plan when the plan differs from an established legal custody agreement or when both parents do not fully agree on the care arrangements.
Active Component Soldiers must complete the Family Care Plan within 30 days after counseling. Reserve Component Soldiers have 60 days. After completing the DA Form 7666 PDF, obtain the required signatures and notarization. Give signed, notarized copies to your unit commander or S1 before departure.
When is Form DA 7666 not required?
You do not need to prepare the DA Form 7666 PDF when the Family Care Plan follows an established legal custody agreement. The form is also unnecessary if both parents agree with the plan and no other person has a legal custody interest.
A Soldier who is married to and living with a civilian spouse generally does not need a Family Care Plan if that spouse can care for the children. In that case, DA Form 7666 is not required.
