For Item 22, report any money or property sold or given away in the past 12 months, if the question applies, and attach an explanation and proof. Read the last page and sign the original form in ink.

When is Form MC 604 IPS needed?

Form MC 604 IPS has no fixed statutory deadline. Your county eligibility worker sets the due date by entering it in the “Return this Form By” field.

Check your copy for that date and return the completed form by the stated deadline. Respond promptly to help prevent delays in processing your eligibility information. If the field is blank or you cannot meet the date, contact your eligibility worker using the phone number listed on the form.

When is Form MC 604 IPS not required?

When your county worker has not specifically requested it, you do not need to complete Form MC 604 IPS on your own. It is also not required if your Medi-Cal case uses only MAGI rules and does not need a non-MAGI review.

From January 1, 2026, the non-MAGI asset limit is $130,000 for one person and $195,000 for two people. These limits apply through June 30, 2027. On July 1, 2027, they are scheduled to fall to $21,000 for one person and $31,000 for two people. Some assets may be excluded.
