When is Form CDPH 530 needed?

Form CDPH 530 has no fixed filing deadline, but California skilled nursing facilities must complete it each day for every shift. Each direct caregiver must sign the form at the end of their shift and record the actual hours worked.

The Director of Nursing or a designee must review and sign the form during the same patient day. The facility must keep each completed sheet as part of its records to support Direct Care Service Hours Per Patient Day reporting.

When is Form CDPH 530 not required?

Form CDPH 530 is not required when normal payroll or timekeeping records fully capture all direct nursing care hours, such as hours from regularly scheduled staff who clock in and out. Use it only for nursing hours that payroll does not capture or hours worked by employees whose main duties are not nursing services.

You do not need to use the CDPH 530 Form PDF if your facility uses its own substantially similar assignment sheet. The form may also be unnecessary when an approved DHPPD waiver covers the applicable 3.5 or 2.4 CNA standard under AFL 18-16 or Title 22 CCR section 72329.2. Facilities that are not licensed as freestanding skilled nursing facilities under HSC section 1250(c) are outside this documentation mandate.
